Custom Jersey Order Form

Ordering Instructions:

Use this form to order one or more custom fishing jerseys. Enter the information in the spaces provided below. (Note: setup fee is per order / same logos for all pieces). When you click Confirm, you will be brought to a confirmation page. When you click Checkout, you will be taken to Paypal to complete the transaction. You don't need to have a Paypal account to complete the transaction. Upon completion of transaction your order will be forwarded to one of our customer service representatives.

Ordering Instructions

Ordering Multiple Jerseys Is Easy!

Each logo configuration must be ordered separately. After completion of the check out process you will receive a message confirming that your order has been sent. You will then have an option to return to the form. The information on the form will remain the same, so you can then enter your next order and only change the information that needs to be changed (first/last names and logo selections, for example). In this way you can enter an entire team and easily keep common logos in the same position on each jersey!

Volume discounts are available for order quantities over 5 and over 20 (not including balaclavas). Please contact us for order quantities over 30.

Select one of our stock designs and choose a color combination, or select "Custom" and describe what you would like in the custom design section.

Select your preferred collar style by clicking on the desired image. Our zippers are black and 4" in length.

Enter names exactly as you want them to appear on the jersey. If you do not want a name on the shirt, simply leave the name section blank.

5. Logos

Enter logo name(s) in desired location(s). Use multiple locations for large logos.

EXAMPLE: For a large Triton logo spanning the chest, enter "Triton" in locations F4 and F5.

NOTE: It is the buyer's responsibility to obtain authorization for the use of any logos.
